What is a Breast Reduction?

A Breast Reduction, also known as a Mammoplasty, is a surgical procedure which reduces the size and weight of the breasts.

Having overly large or heavy breasts can cause a number of physical and emotional concerns:

  • Muscular strain – Neck, shoulder and back pain can result from the excess weight placed on the body
  • Poor posture - Heavy breasts can pull the body weight forward, affecting posture and comfort
  • Clothing fit and comfort – Many individuals find that getting clothing and underwear that fits well is a challenge
  • Mobility restriction – Movement can be heavily limited, making exercise and daily activities hard to undertake
  • Skin irritation – Some people may be affected by chafing or skin irritation underneath the crease of the breast (inframammary fold)
  • Loss of confidence – Impacted physical and emotional comfort takes a toll on quality of life, causing a loss of confidence and increased self-consciousness

Large or heavy breasts can occur for many reasons and aren’t always connected to overall weight. Especially in younger individuals, overly large breasts are caused by hormonal imbalances during puberty.

The functional and aesthetic concerns arising from having overly large breasts can have a significant impact on confidence, comfort and quality of life, and it is normal for people to seek an effective solution as soon as possible. If you are considering Breast Reduction Surgery, it is important to understand the factors your surgeon will consider when determining if you meet the age requirements for Breast Reduction and what age for Breast Reduction is considered safe and appropriate.

Between 2023 and 2024, the British Association of Aesthetic Plastic Surgeons recorded that Breast Reduction was the second most popular procedure carried out in the UK.

Cadogan Clinic has brought together a team of the UK’s leading Consultant Breast Surgeons who have performed several thousand Breast Reductions between them.

How Old Can You Be to Get a Breast Reduction?

When determining if you are a suitable candidate for Breast Reduction, age is one of several considerations your surgeon will take into account. The minimum age for Breast Reduction surgery in the UK depends on why the surgery is being carried out and how it will benefit the patient.

In the UK, the legal age for all cosmetic surgery procedures is 18. This means that it is illegal for any surgeon to perform a Breast Reduction for purely aesthetic reasons on a patient under the age of 18. This would include reasons such as changing the size and shape of the breasts, without any medical benefit to the patient.

However, for patients whose breast size is impacting their comfort and quality of life, or restricting their ability to undertake daily activities, it may be possible to perform a Breast Reduction at a younger age to improve and protect health and well-being.

There are two main reasons for the UK Breast Reduction age:

Physical Development

To successfully perform a Breast Reduction, and achieve the best results, the breasts must be fully developed. The size of the breasts can vary through life for many reasons, so size may not be the only determining factor in whether they are fully developed.

Typically, the breasts are fully developed at around the age of 17, between two and five years after growth begins. However, for some individuals, this may be earlier, or in some cases, delayed into their early 20s.

Whether Breast Reduction is being performed for cosmetic or functional reasons, it is important that the breasts are fully developed to ensure that they are not damaged or impaired.

Emotional Development

In UK law, an individual is considered an adult and capable of making decisions about their own body at the age of 18.

Many patients find that undergoing surgery has a positive and beneficial impact on their confidence and quality of life. However, it is important to be able to understand the potential risks and side effects of any procedure you are considering.

Breast Reduction for Medical Reasons in Younger Patients

Medical reasons that a surgeon may decide that a patient is suitable for Breast Reduction under the age of 18 include:

Gigantomastia

Gigantomastia, also known as Macromastia, is a rare medical condition which is characterised by the dramatic, excessive growth of the breasts.

Rather than just being a case of large breasts, Gigantomastia is caused by rapid tissue hypertrophy, where the breasts grow at an unexplained rate, which can vary from a few weeks to years.

To be diagnosed with Gigantomastia:

  • The breasts should be made up of at least 2.2kg (5lbs) of breast tissue each
  • Excess breast tissue equals more than 3% of the total body weight

To effectively treat Gigantomastia with Breast Reduction, your consultant will need to establish the exact cause. In cases where it is hormonal, it may be necessary to control the hormones with medication before surgery can take place, to prevent the condition from recurring. This being said, many patients find that Breast Reduction is a highly effective solution and increases comfort and quality of life dramatically.

Severe Muscular Strain

Severe neck, shoulder and back pain caused by overly large breasts can have a significant impact on quality of life. Many patients find they are in constant discomfort, which can affect their ability to work and socialise, and may also impact sleep comfort, further affecting overall wellbeing.

Reducing the size and weight of breasts with Breast Reduction surgery can be an effective treatment for associated concerns, such as musculoskeletal pain, providing a long-term improvement in comfort and quality of life.

However, your surgeon will wish to establish that the breasts are fully developed in order to ensure that the surgery isn’t going to impair future breast function.

Mobility Limitations

Having overly large breasts can affect mobility for two reasons:

  • The actual size of the breasts limits the field of motion, especially in the torso and arms
  • Muscular pain associated with breast size means that movement is uncomfortable

As well as making exercise challenging, mobility limitations can have deeper implications for overall health and affect participation in typical daily activities, such as driving or household tasks.

In cases where a patient’s daily quality of life is significantly affected by mobility limitations, it may be possible to reduce the usual boob reduction age to address their concerns.

Chronic Skin Irritation and Infections

The size and weight of the breasts can cause skin irritation and chafing, especially under the inframammary fold (the crease under the breasts). Moisture can become trapped here, leading to skin rubbing and potentially allowing bacteria to multiply, causing skin infections.

Skin irritation and infections can be uncomfortable and lead to embarrassment. Where the underlying cause (in this instance, the size of the breasts) is not addressed, it can be challenging to effectively address the skin concerns and infections. When chronic, these concerns can have a deeper impact on health and well-being.

In cases where a younger patient is deemed suitable for a Breast Reduction, and there is a medical benefit in reducing skin irritation and preventing future infections, it is often an effective solution.

Breast Reduction Age Requirements

In the UK, the guidance for surgeons is that a Breast Reduction shouldn’t be performed on a patient under the age of 18 unless it is in their best interest. Typically, this means that the benefits of the procedure significantly outweigh:

  • The potential risks of the surgery itself
  • The risks of not performing the procedure at that specific time

Your concerns, the consultant’s assessment, and their discretion will all be considered when deciding whether you are suitable for teenage Breast Reduction.

There may be other policies and criteria which need to be fulfilled ahead of your Breast Reduction:

  • Your surgical plan may need to be approved or signed off by a second consultant to ensure that your concerns have been assessed thoroughly and correctly
  • Your surgeon may ask you to supply a letter from your GP or a medical professional you have spoken to about your breast concerns to ensure that all treatment options have been considered
  • If you will be under the age of 18 at the time of your procedure, you will need written parental permission

In some cases, a surgeon may be happy to see you for a consultation before you turn 18, with the intention of carrying out the procedure once you meet the Breast Reduction age requirements.

What is the Best Age to Get a Breast Reduction?

Although the earliest age for Breast Reduction for cosmetic reasons is 18, adult patients of any age often consider and undergo the procedure, meaning there isn’t necessarily a best age to get Breast Reduction surgery.

For patients over 18, age is unlikely to be specifically considered when determining whether they are suitable for Breast Reduction. However, there may be certain considerations around other factors in their life which influence the eligibility criteria:

  • Weight Loss – For patients who are currently undergoing dramatic weight loss, your surgeon will ask that you have reached and maintained your ideal weight for at least 6 months before undergoing Breast Reduction
  • Family PlanningPregnancy and breastfeeding may impact the results of your Breast Reduction, and depending on your concerns, your surgeon may suggest delaying a Breast Reduction until you have completed your family
  • Hormonal Changes – If your breast size is associated with a hormonal imbalance, it is important that stability has been reached before undergoing Breast Reduction, to prevent the breast tissue from regrowing

Alongside these factors, there are other standard considerations your surgeon will take into account:

  • Your concerns – Your surgeon needs a comprehensive understanding of your concerns and reasons for considering Breast Reduction surgery to ensure it is an effective procedure for you
  • Your desired results – It is important that you understand the realistic results that are possible for you from a Breast Reduction before going ahead with the procedure
  • Your medical history – To ensure you are fit and well for a general anaesthetic procedure, your surgeon will discuss your medical history, including any underlying medical conditions

Frequently Asked Questions

In the UK, the minimum age for Breast Reduction surgery is 18, in line with the laws around cosmetic surgery. However, in some cases where overly large breasts are causing medical concerns, such as musculoskeletal problems, or chronic skin infections, it may be possible to carry out the procedure at a younger age if you meet the eligibility criteria and it is determined that the benefits outweigh the risks.
