What is Facial Feminisation Surgery?

Facial Feminisation Surgery, often abbreviated to FFS, is a combination of surgical procedures which create a more feminine appearance for transgender women. These procedures enhance the shape and proportions of different areas of the face to alter the facial secondary sexual characteristics from male to female.

The exact procedure, or combination of procedures, recommended for you by your consultant to make up FFS, will depend on your individual concerns. You may opt to have the procedures carried out as part of the same surgery, or over a period of time, depending on what suits your needs.

For transgender women, having typically male facial characteristics such as a square jaw, thin lips, broad nose or a tall forehead, may cause or increase feelings of gender dysphoria – the feeling that your assigned sex at birth doesn’t fit how you feel about and see yourself. As the face provides other people with the first impression of how we present ourselves, Facial Feminisation Surgery can be a very important step in gender transition for many patients.

Not every transgender woman will choose to undergo Facial Feminisation Surgery and it is not essential to have every element of the surgery. FFS is offered as a bespoke surgery to suit your needs.

Benefits of Facial Feminisation Surgery

Facial Feminisation Surgery is a highly beneficial surgery for transgender women, with some patients feeling it is more important than other gender reassignment surgeries, as it enables them to integrate socially more easily.

A more feminine facial appearance – FFS’s primary purpose is to create a more feminine shape and proportions to the face. This may involve procedures altering the jawline, Adam’s Apple, lips, cheeks, nose, forehead and brows. As these procedures are surgical, the results will be long term.

Decreased gender dysphoria – For transgender women, having typically male facial features can result in serious feelings of gender dysphoria. Anxiety around how they are viewed by others and whether they are able to integrate socially can worsen these feelings. Facial Feminisation Surgery can help to address these feelings and reduce gender dysphoria. As the surgical results are long term, maintenance appointments are not needed every few months, which can also impact gender dysphoria as results are lost in a shorter timeframe.

Who is Suitable for Facial Feminisation Surgery?

Your consultant will assess whether you are suitable for Facial Feminisation Surgery during your in-person consultation at our award-winning clinic based in Chelsea, London. They will consider a number of factors to determine whether you are a suitable candidate for surgery.

Your consultant will discuss your concerns and examine your face to assess which procedures may be appropriate as part of your Facial Feminisation Surgery. They will also explain the results that are possible from FFS for you.

They will also discuss your medical and mental health history to make sure you are fit and well for a general anaesthetic procedure – this may include your blood pressure, height and weight as well as any medical conditions you have.

You must be over 18 to have Facial Feminisation Surgery at Cadogan Clinic. Your consultant may also require referrals from other gender reassignment specialists to ensure you are at the best stage in your transition for a Facial Feminisation procedure.

What Does Facial Feminisation Surgery Involve?

Facial Feminisation Surgery can involve a combination of procedures which alter the shape and proportions of the face and neck. The procedures that your consultant recommends will be bespoke to you and your concerns. These procedures may be carried out as part of the same surgery, or in different surgical sessions, depending on the procedures and what works best for you. 

Although both men and women have a lump in the throat, known as the Adam’s Apple, this is usually much more prominent in men. The appearance of the Adam’s Apple can be reduced by shaving the thyroid cartilage in the neck. This results in a smoother, more elegant appearance to the neck. 

For patients who are concerned they have drooping eyebrows or forehead lines, a Brow Lift as part of FFS can smooth the upper portion of the face and raise the position of the eyebrows giving a feminine and youthful appearance. Brow Lift surgery specifically targets the upper portion of the face, which other facial procedure aren’t able to do as effectively. 

To create a more feminine face shape with high cheekbones, some patients may benefit from Cheek Enhancement or Augmentation as part of FFS. This can be achieved either with cheek implants, or using Fat Transfer, depending on the results you are hoping to achieve. 

To reduce the size of a prominent chin or soften a square chin, Chin Contouring can be carried out to enhance the chin area. This may involve techniques which reduce the amount of bone in the area or use options such as Fat Transfer to change and soften the shape of the chin. 

Typically, the female eye has a slight downward slant in the inner corner, as well as being slightly larger with higher upper eyelids – creating more visible space between the eye and brow. Eye Feminisation surgery creates a more open, lifted appearance to the eyes adding to a feminine appearance. 

A Facelift can be carried out as part of Facial Feminisation Surgery to reduce the appearance of ageing male features. This may include jowls around the jawline, sunken or flat cheeks, and drooping eyelids. This may then be intensified by a natural loss of underlying fat, muscle and bone which can create a sunken appearance. 

Many transgender women are concerned that they have an overly prominent brow ridge (the bony ridge above the eye sockets) and that their forehead may be too flat. Forehead and Brow Contouring results in a flatter brow ridge, with a more rounded, narrower forehead. 

Also known as Forehead Reduction, Hairline Lowering involves adjusting the height of the forehead, to make it smaller. By removing a portion of the skin at the top of the forehead and moving the position of the hairline down to close the incision the hairline is lowered, resulting in a smaller forehead. 

Whereas typically a male jawline is quite square and broad, a female jawline is softer and rounder. By reducing the bone in the jawline, or using Fat Transfer to soften the shape of the jawline the result is a more feminine jawline which adds to the overall feminine face shape. 

For patients who aren’t happy with the fullness of their lips, Lip Augmentation can increase the size of the lips, using either Dermal Fillers or Fat Transfer. Dermal Fillers offer a temporary solution, which requires maintenance every 6-12 months. However, some patients opt for Dermal Fillers to understand the results they may be able to achieve from Fat Transfer as a permanent option. 

Neck Lift is carried out to reduce any sagging skin in the neck and improve the contours of the neck. Ageing can cause the skin around the neck to sag and masculine features may be exacerbated with this. A Neck Lift during FFS removes the excess tissue around the neck and tightens the muscles to create a slender, elegant appearance. 

Typically, a female nose is smaller and slimmer than a male nose. Rhinoplasty surgery can be carried out as part of Facial Feminisation Surgery to reduce the overall size of the nose and create a more feminine appearance.  

The upper lip in men tends to be thinner, with a flatter shape, whereas women have a more defined cupid’s bow. By removing a small amount of tissue from below the nose as part of an Upper Lip Lift, above the upper lip, the lip is lifted, enhancing the shape of the lip, and balancing the size of the upper lip with the lower lip.

What to Expect

Cadogan Clinic are at the forefront of surgical excellence and our dedicated team are proud to provide the best surgical journey for every patient. 

When you contact us, one of our dedicated surgical patient advisors will answer any questions you may have and book a consultation for you with one of our Facial Feminisation specialist consultants.

You will then meet with your consultant at our clinic on Sloane Street in Chelsea. During this consultation, they will discuss your concerns and examine your face and neck. They will assess whether you are suitable for FFS and explain which the best procedures are to address your concerns.

If they decide that you are suitable for FFS, your consultant will send your treatment plan through to your patient advisor, who will be able to discuss the final price and schedule a date for your surgery. 

If you decide to proceed with your Facial Feminisation Surgery, your consultant will ask you to prepare for your surgery in the following ways:

  • You will need to complete your pre-operative paperwork to allow your surgical team (surgeon, anaesthetist and nurses) to review your surgery plan and advise on any final changes you need to make in the run up to your surgery. 
  • You must stop smoking and vaping for six weeks before surgery can take place. This is vital to ensure your recovery is smooth and you heal effectively, achieving your desired results. 
  • You will be asked to cut down on drinking alcohol in the weeks running up to your surgery. You must avoid drinking any alcohol for 48 hours before your surgery. This is to ensure you recover well and to minimise underlying inflammation. 
  • You will need to stop taking any anti-inflammatory medications such as ibuprofen and aspirin, as well as any herbal supplements as these can increase the risk of bleeding. Your consultant will advise on the timeframes for stopping any medication as this can vary between patients. 
  • You will need to organise for a friend or family member to collect you after your procedure. They must be able to drive you themselves or accompany you in a taxi before staying with you overnight after your Facial Feminisation Surgery. 
  • You will be asked to fast before you arrive at the clinic. You mustn’t eat any solid food or consume any juice or milk (including tea or coffee) for 6 hours before you arrive at Cadogan Clinic for your surgery. You can have sips of clear liquids (water, or black tea and coffee) up to 2 hours before you arrive at the clinic. You mustn’t have anything in the 2 hours before you arrive. 

On the day of your surgery, you will arrive at the clinic an hour before your procedure is scheduled to begin. Our specialist nurses will carry out some post-operative checks such as your temperature, blood pressure and heart rate and will ask you to put on a surgical gown and compression stockings.

Your surgeon and anaesthetist will come and speak to you and answer any final questions you may have before your FFS. Your surgeon may then make some markings in the areas they are operating on.

You will then be taken into one of our state-of-the-art operating theatres. The theatre team will make sure you are comfortable, and your anaesthetist will administer your general anaesthetic. All surgical procedures at Cadogan Clinic are carried out with Trans Intravenous Anaesthetic (TIVA) which lowers the risk of post-operative nausea and sickness and speeds up your initial recovery time.

Your consultant will then complete your Facial Feminisation Surgery. Depending on the exact procedures they are carrying out they will close any incisions with stitches and apply dressings to protect the areas.

You will wake up in our recovery ward. One of our specialist nurses will monitor your heart rate, blood pressure and temperature to make sure you are recovering well. When you have woken up and you’re comfortable, you will be taken to our ambulatory ward. Here you will have something to eat, and the nurses will prepare you to go home. When you are ready, they will explain your aftercare instructions and call your chaperone to come and collect you. You will then be able to leave to recover in the comfort of your own home. 

Recovery time following Facial Feminisation Surgery can vary depending on the procedures that your consultant has carried out. It is normal to experience bruising and swelling straight after your surgery, and this can take 2-3 weeks to subside. Most patients are mostly recovered within 6-8 weeks, however in some cases it may take up to a year to see the final results.

During your recovery, your consultant will ask you to avoid any strenuous activity or exercise to prevent putting strain on the surgical areas. You may also need to sleep on your back, or slightly propped up.

One week after your FFS surgery, you will come back to the clinic to see our nurses, so they can assess your incisions and make sure you are recovering well. At this stage they will change your dressings and remove any stitches, as well as making sure you are confident in how to look after your dressings and incisions going forwards.

Six weeks after your procedure, you will meet with your consultant to discuss your recovery and results. At this stage they will let you know when you can return to strenuous activities or exercise, and they will also explain what you can expect from the next stages of recovery.

You will have 24/7 access to our nurses to ask any questions or raise any concerns for up to 2 years following your FFS surgery at Cadogan Clinic.

Both men and women have an Adam’s Apple, which is the cartilage that covers the front of the voice box. However, this is more prominent in men, resulting in a visible bump in the front of the neck and also leading to them having a deeper voice. FFS Surgery can include Adam’s Apple Reduction to reduce the appearance of the Adam’s Apple and smooth the neck.
