Asymmetrical nostrils have many causes, and understanding the cause of your uneven nostrils is crucial in determining the best treatment. Genetics
Our genes determine many of our features. Some people may be predisposed to having one nostril bigger than the other or different shaped nostrils due to their natural nostril anatomy. Injury or Trauma
Injury to the nose is one of the most common causes of asymmetrical or different shaped nostrils. Appearance
For some people, the difference in the size and shape of their nostrils can be quite clearly seen when looking at the front, profile or underside of their nose (known as the alar base). This may not always be deviated septum symptoms, but a nasal septum deviation can sometimes be seen when looking at the alar base. The edges of the nostrils can also be slightly higher than looks proportionate, this is known as Alar Rim Retraction.
Blockage in nostrils
Having the feeling of both or one nostril blocked over a long period of time can be a sign that you have asymmetrical nostrils. This may also lead to breathing difficulties or snoring and sleep apnoea. You can tell if you have both or one nostril blocked as your nose will feel stuffy, similar to having a cold or allergies. However, the blockage won’t improve over time, or when you blow your nose.
Recurrent nosebleeds
A common sign that one nostril is bigger than the other, and that there may be an obstruction within the nose is recurrent nosebleeds. This can be due to ongoing nasal irritation or dryness.
Dry mouth
Having a dry mouth, especially when you wake up in the morning, is a sign of breathing difficulties, meaning you are favouring breathing through your mouth, especially when you are asleep. Breathing through your mouth when you are asleep also increases the risk of snoring.
Being aware of the nasal cycle
The dominance of the airways in the nose naturally alternates roughly every 2-3 hours. This maintains the essential humidity within the airways and the sense of smell. If you are aware of the airways switching sides throughout the day, it may be that one nostril is larger than the other.
Sleeping position
Having a preference for sleeping on one side may indicate that you have one nostril bigger than the other. You may find breathing easier on your preferred side, even if you aren’t aware of the difference.
Fortunately, there are many solutions to address one bigger nostril or different shaped nostrils. The right treatment for you will depend on the cause and severity of your concerns and whether they are only cosmetic, or functional as well.
Non-surgical treatments are an effective way of addressing the size of asymmetrical nostrils and changing the nostril shape. However, non-surgical treatments are only able to address cosmetic concerns, and cannot improve breathing difficulties or other functional issues.
A Non-Surgical Rhinoplasty involves injecting small amounts of Dermal Filler into the soft tissues of the nose to change its shape. This includes around the corners of the nose and the nostrils to adjust their size and shape. A Non-Surgical Rhinoplasty usually lasts between six and twelve months, before a follow-up appointment is required to maintain the results.

Alar Base Reduction, also known as Nostril Reduction Surgery, is a procedure which focuses on changing the size and shape of the nostrils. For patients concerned about the position of the edge of their nostrils (known as the alarm rim), grafts can be placed to lower the edge and create an even shape and size. Tissue can also be removed from the nostrils to create a consistent shape and reduce nostril flare.
Alar Base Reduction can also be combined with other nose surgery procedures to enhance the final results and address other areas of the nose.
For patients who also have functional concerns or who have a deviated septum, Septorhinoplasty is a comprehensive and effective surgery. The nose is opened to allow the surgeon to work on the internal structures of the nose, straightening any crooked cartilage and opening the airways.
