What is Buccal Fat Removal?

Buccal Fat Removal is a cheek contouring procedure which slims the lower face, and accentuates the cheekbones, creating a defined and contoured appearance.

Buccal Fat Removal involves:

  • A small incision is made on the inside of each cheek
  • The entire buccal fat pad is removed from the lower cheek, above the jawline
  • The incisions are closed with dissolvable stitches

To emphasise the desirable ‘V’ shape which many individuals are aiming to achieve from Buccal Fat Removal, other procedures such as Chin Liposuction may be performed at the same time.

Buccal Fat Removal can be performed under local or general anaesthetic, and usually takes less than an hour to complete.

What is Dimpleplasty?

Dimpleplasty is also a cheek contouring procedure, which creates the appearance of dimples, small indentations in the cheeks, which appear when smiling. Dimples can enhance facial expression and give a distinctive appearance.

To create dimples, during Dimpleplasty:

  • A small incision is made on the inside of each cheek
  • The underside of the cheek skin is stitched to the underlying muscle using dissolvable stitches
  • The incisions are closed using dissolvable stitches.

Dimpleplasty can be combined with other facial procedures to achieve a comprehensive rejuvenation of the lower face:

Dimpleplasty is usually performed under local anaesthetic and takes less than an hour to complete.

 


Cheek Anatomy

Understanding the natural anatomy of the cheeks helps to show how Buccal Fat Removal and Dimpleplasty work.

The cheeks are made up of several muscles and fat pads, which provide facial structure, influence facial shape, and enable movement and expression.

Buccal Fat Pads

  • Located in the lower part of the cheeks, above the jawline
  • Influence the shape of the face
  • Provide structure and protection to the lower face and mouth

Natural Dimples

  • Occur due to the zygomaticus major muscle in the cheeks dividing into two, near the mouth, due to a genetic trait (known as a bipid zygomaticus major)
  • It is important to note that Dimpleplasty does not involve operating on this muscle, but the same appearance can be created by joining the skin and muscle within the cheek.

Buccal Fat Removal Recovery

  • Food – To protect the incisions inside the mouth, it is important to eat only soft and liquid foods for up to 2 weeks after surgery
  • Wound care – It is essential to keep the area clean to prevent infection
  • Exercise – You should avoid strenuous activity and exercise for at least 4 weeks after surgery
  • Full recovery – You should be fully recovered within 4-6 weeks

Dimpleplasty Recovery

  • Swelling and bruising - Swelling and bruising around the cheeks is normal and typically heals within 10-14 days
  • Food – Your surgeon will request that you only eat soft and liquid foods for the first 2 weeks of your recovery to protect the incisions
  • Wound care – You will be given instructions on keeping the surgery area clean to prevent infection
  • Exercise – You should avoid exercise and strenuous activity for at least 4 weeks after surgery
  • Full recovery – Full recovery after Dimpleplasty usually takes 4-6 weeks

As Buccal Fat Removal addresses the facial fat pads, it won’t change the shape or structure of the muscles which create the appearance of cheek dimples.

However, dimples after Buccal Fat Removal may have an altered appearance or depth as the procedure changes the shape and contours of the cheeks. If you are concerned about the appearance of your dimples changing, it is important to discuss this with your surgeon during your consultation.

Dimples result either from the zygomaticus major muscle being naturally divided into two, or through the skin and muscle in the cheek being joined together surgically.

Removing the buccal fat pads from the cheeks doesn’t change the structure of the cheeks’ skin and muscles and therefore won’t result in dimples. However, removing the fat pads does change the contours and definition of the lower face, and this may impact expression in a similar way to cheek dimples.

As both Buccal Fat Removal and Dimpleplasty are performed through an incision in the inner cheek, it may be possible to combine the procedures to combine Buccal Fat Removal and dimple creation.

This will vary between patients, depending on what is involved to achieve successful outcomes in each procedure, and it is important to speak with your surgeon to understand if a combined Buccal Fat Removal and dimple creation procedure is possible for your concerns.
