What is Penis Enlargement surgery?

Penis enlargement surgery is a procedure which aims to increase the length or width (girth) of the penis. It generally adds one to two inches to the length of the penis. Many patients opt to have their girth and length increased at the same procedure.

The surgery works by basically ‘freeing up’ a ligament which will cause the penis to move forward and drop down, adding length to the penis when it is in its flaccid state. In order to increase the length of the penis in its erect state, after the surgery the patient will be given a penile extender, which will help to increase the length of an erection over the following 12 months. Alternatively, medical-grade silicone can be implanted under the penile skin to make the penis longer.

In order to increase the girth, a procedure called fat grafting will see fat cells extracted from another part of the body and processed before being injected into the penis.

Some men also decide to undergo a circumcision (foreskin removal) at the same time as their penis enlargement surgery as an increase in girth can make the foreskin feel overly-tight.

The results of penis enlargement surgery are permanent. It is the most effective way to increase the size of your penis.

Why do people have it?

Men choose to have penis enlargement surgery because they are dissatisfied with the length or girth of their penis. Often this is because the men feel self-conscious about their appearance, which may prevent them from forming intimate relationships. Although sexual intercourse may or may not improve following penis enlargment surgery, just this boost in confidence can help to make sex a more relaxed and pleasurable experience for both parties.

This self-consciousness can also lead to men avoiding certain activities, such as swimming and going to the gym or other activities that involve using a communal changing room. Often these feelings stem from insecurity, rather than any physical problem with the size of their penis.

Sometimes a buried penis or micropenis, a congenital problem where the penis measures less than three inches when errect, can make penetrative sex impossible. Penis enlargement surgery can help with this.

Who is suitable?

Cadogan Clinic considers individuals fit for penis enlargement, if all of the following are true of them:

  • Physically and psychologically fit and healthy

How does the procedure work?

Penis enlargement surgery is a straightforward procedure and is carried out at the Cadogan Clinic as a day case, meaning you will be able to go home on the same day as your surgery.

The surgeon will make an incision at the base of your penis to gain access to the suspensory ligament, which is attached to the pubic bone. The surgeon will then proceed to carefully divide the suspensory ligament, causing the penis to move forward and down, gaining around one to two inches in length. A silicone buffer will be inserted to ensure that the separated ends of the ligament do not rejoin.

Although the penis appears longer because more of it is hanging down, this does not actually alter the length of the penis - it merely rearranges what was already there. This increase in length can only be seen when the penis is flaccid. Once a patient has recovered from surgery, a penile extender can add about one inch in length to an erect penis if used as directed for 12 months. The penile extender should be worn for one to two hours every day during this period.

An alternative penis enlargement surgery involves inserting a piece of medical-grade silicone into the penis to make it longer and wider. To do this, the surgeon will make a small cut above the penis where the silicone will be slipped through to fit snugly within the shaft of the penis.

If the aim is to introduce girth to the penis, this is done using a procedure called fat transfer. Also known as a fat graft or lipofilling, fat transfer is a surgical treatment which involves moving fat from one part of the body to another. Liposuction is used to suck out unwanted excess fat cells from one part of the body, such as the thighs or abdomen. These fat cells are then treated before being reintroduced to a different part of the body, in this case, the penis. With fat transfer, penis girth will increase by one to two inches, when both flaccid and erect.

After a fat transfer to penis, you may wish to consider circumcision to remove your foreskin. This is because the increase in girth may make your foreskin feel overly-tight, which can cause difficulty and discomfort when urinating or during sexual intercourse.

What kind of results can I expect?

Cutting suspensory ligament, causing the penis to move forward and drop down, can lengthen the appearance of the penis by one to two inches when flaccid only. Using a penis extender for 12 months after surgery can add around one inch to an erect penis. Fat transfer to increase penis girth can add around one to two inches.

A silicone implant can add an extra one-and-a-half to two-and-a-half inches to penis length when flaccid and erect. The implant will make your penis look both longer and wider, although there are greater risks attached to this procedure.

Penis enlargement surgery is permanent.

It is important to have realistic expectations as to what can be achieved during penis enlargement surgery.

In some cases, for example for some men who were born with a micropenis, alternative surgery, such as phalloplasty, may be a better option.

What to expect

Below you will find some key information to help you prepare for surgery and make necessary arrangements to ensure you have the support you need and that everything runs smoothly on the day.

Your journey with the Cadogan Clinic will start with a consultation with one of our leading specialist plastic surgeons at our award-winning premises in Chelsea. These consultations will take between 30 and 60 minutes.

During your consultation, your surgeon will ask about your expectations for surgery and talk about what will happen during the procedure. They will also explain how the procedure will affect you.

It is important that you are open and honest about what you hope to achieve through surgery to help ensure the best possible result. Your surgeon will be happy to answer any questions you may have at this point. It may be a good idea to write your questions down before attending the consultation. Having established what you want from the surgery, and what is possible given your anatomy, several key measurements will be made and standard clinical photography will be taken. Your medical history will be discussed and a physical examination will be carried out.

If your surgeon feels you are a good candidate for surgery, you are then invited to consider whether you would like to proceed with your surgery or not following a two week 'cooling off' period.

Avoid smoking and drinking for 48 hours ahead of your penis enlargement as this can delay the healing process. Avoid taking aspirin, anti-inflammatory drugs and herbal supplements in the week ahead of surgery.

In the six hours prior to surgery you must not consume food or any drink, other than small sips of still water, black coffee or black tea, which will be allowed up to two hours before you are admitted to the clinic.

You will be given a general anesthetic to put you to sleep, so you will not be able to feel any pain during the procedure. Once you are asleep, the surgery will proceed to make an incision in your penis and either insert the silicone into the shaft or divide the suspensory ligament, depending on which method is being used. The incisions will be closed using dissolvable sutures.

Following your penis enlargement surgery, you will recover in our ambulatory recovery rooms. You may feel a little drowsy but this often wears off within a few hours.  Once you are able to eat and drink, and our specialist nursing team are happy that your initial recovery is complete, you will be allowed to leave the clinic with a friend or family member. Someone should stay with you for 24 hours after surgery.

You should expect to take up to a week off work following your penis enlargement surgery. All sexual activity, including masterbation, contact sports and the gym should be avoided for six weeks after surgery. Once this healing period has finished, you may start using the penile extender, if appropriate.

It takes around six weeks to fully recover from penis enlargement surgery. This includes circumcision if you opted to have this procedure alongside your surgical penis enlargement.

You may take up to a week off work following your penis enlargement surgery, but if you have a sedentary job, for example, and office job, then you may find that you can return within two or three days. Avoid heavy lifting for four weeks and avoid sex, contact sports and the gym for six weeks. Failure to do so may result in injury and affect the final result.

Penis enlargement surgery can add an extra inch or two on to your penis length, but the exact length can never be guaranteed. It is important that you enter into this surgery with realistic expectations as to what can be achieved.

Sometimes an alternative treatment, such as phalloplasty, may be more appropriate. Your surgeon will discuss the options with you at your initial consultation.

There are several non-surgical penis enlargement options. At best, most of these won’t work. At worst, they may cause lasting damage to your penis.

You may have already seen pills and lotions on sale, particularly online, which claim to give you a larger penis. These may contain vitamins, herbs or hormones. However there is no scientific evidence to demonstrate that these remedies work.

Another penis enlargement ‘treatment’ that you may have seen advertised online is a vacuum pump, or penis pump. This device draws blood into the penis, causing it to swell and look larger. But the effect is only temporary and if used incorrectly, or for a prolonged period of time, this can damage delicate elastic tissue in the penis. If this tissue is damaged, then you will find that your erections are less firm.

Specific exercises, designed to force blood from the base to the tip of the penis, have not been proven to work either. These may also lead to the penis becoming disfigured or cause pain or discomfort. A stretching device may be used to help add an inch to an erect penis. However this takes a long time, some men find it uncomfortable, and it does not always work.

Penis enlargement surgery is the safest and most effective way to improve the length and girth of your penis permanently.

It is common to experience swelling, pain and redness in the days following penis enlargement surgery. Other risks and side effects include:

  • Developing a bend or kink in the penis
  • Scarring
  • Infection
  • Erectile dysfunction
  • Loss of sensation in the penis
  • Needing further surgery which may leave the penis shorter

General complications from any type of surgery include pain, scarring, blood clots (deep vein thrombosis) and an adverse reaction to the anaesthetic.
