Mons pubis reduction surgery, also known as monsplasty or mons lift, is an invasive medical procedure to remove the excess skin and fatty tissue from the fatty pad sitting on top of the pubic bone. This fatty pad is known as the mons pubis.
Sometimes the mons pubis can get bigger due to the accumulation of excess fat, or you may find you have a lot of loose skin covering the area. Mons surgery successfully removes any excess fat and skin, leaving behind a flatter, firmer mons pubis.
The form mons reduction can take varies according to the issue that is being treated. If excessive lax skin is the problem, then the surgeon will cut away this excess skin. If fat accumulation is the issue, then mons pubis liposuction will be carried out, followed by skin excision if required.
If you need liposuction as part of your mons pubis reduction, you won’t always need skin excision afterwards. If you have only been carrying this excess weight for a short period of time and you are young then the skin should have enough elasticity to return to its original shape. If you are having a lot of weight removed, or if you are older with reduced skin elasticity, then you will find that excess sagging skin will be left behind after your mons pubis liposuction.
It is common for mons surgery to be carried out at the same time as a tummy tuck (abdominoplasty). This is because a tummy tuck tightens the skin on the abdomen, therefore making a fatty mons pubis appear bulging and more noticeable.

For many people, mons pubis reduction is all about improving the aesthetics of the area. However an enlarged mons pubis can also interfere with the function of the genitals. If excess sagging skin is causing the mons pubis to droop down in front of the urethra and vulva, then this can interfere with urination and make sexual intercourse difficult or painful.
As with any part of the body where excess folds of skin are present, people with an enlarged mons pubis may find it difficult to maintain a good level of personal hygiene, particularly around the genital area. A lapse in personal hygiene in this area can lead to further health problems.
Women who have previously undergone a C-section, or other surgery in the lower abdomen, may find that the skin around the scar may have remained tight against the tissues, but the skin below it hasn’t, resulting in a noticeable bulge. Women who have just given birth, along with anyone else who has suddenly lost a large amount of weight, may find they are left with a large amount of excess skin around the stomach, requiring a tummy tuck. Mons lift surgery is often carried out at the same time to prevent the area from looking overly prominent or out of place.
Mons pubis reduction surgery can also help to improve mental wellbeing and boost self esteem.

Mons reduction surgery works by reducing the size of an enlarged mons pubis, making it appear less prominent, therefore improving the overall aesthetic of the area. It also alleviates any problems a drooping mons pubis may cause with urination and sexual intercourse. But what are the options for mons reduction?
Mons pubis liposuction
If you have excess fat stored around the mons pubis, then liposuction will be carried out as part of your mons surgery. Liposuction is a cosmetic surgery procedure used to remove stubborn fat deposits. A cannula is inserted into the treatment area and these fat cells are broken up by way of vibrations or high pressure water jet before a vacuum machine is used to loosen and suck out the fat.
Although liposuction is a highly effective method of fat removal, it does not get rid of excess skin. If you have poor skin elasticity, then you will need any loose or sagging excess skin to be surgically removed.
Mons pubis surgery without liposuction
If you have lost a large amount of weight, it is common for a large amount of loose excess skin to be left behind. This is because the expanded fat cells stretch the skin, and when these fat cells have shrunk, there is often not enough elasticity present for the skin to spring back to where it is supposed to be.
If loose skin only is the problem, then the surgeon will carry out a mons lift using a scalpel to cut away excess skin. The mons pubis will be sculpted by tightening the underlying tissues and muscle.

Below you will find some key information to help you prepare for surgery and make necessary arrangements to ensure you have the support you need and that everything runs smoothly on the day.
01.The Consultation
Your journey with the Cadogan Clinic will start with up to two consultations with one of our leading specialist plastic surgeons at our award-winning premises in Chelsea. These consultations will take between 30 and 60 minutes.
During your consultation, your surgeon will ask about your expectations for your mons pubis reduction and talk about what will happen during the procedure. They will also explain how the procedure will affect you and the recovery period.
It is important that you are open and honest about what you hope to achieve through surgery to help ensure the best possible result. Your surgeon will be happy to answer any questions you may have at this point. It may be a good idea to write your questions down before attending the consultation. Having established what you want from the surgery, and what is possible given your anatomy, several key measurements will be made and standard clinical photography will be taken. Your medical history will be discussed and a physical examination will be carried out.
If your surgeon feels you are a good candidate for surgery, you are then invited to consider whether you would like to proceed with your mons lift or not following a two week 'cooling off' period.
02.Leading up to Procedure
If you decide to go ahead with surgery, the next time you will return to the clinic is on the day of your mons lift. It is important that you prepare by taking the following measures ahead of surgery.
In the six hours prior to surgery you must not consume food or any drink, other than small sips of clear fluid which will be allowed up to two hours before you are admitted to the clinic. Clear fluids include still water, black coffee and black tea.
03.On the day
Your mons surgery will take place at our award-winning clinic in Chelsea. You will be given a general anaesthetic to put you to sleep for the operation. In some cases the surgeon may use a local anaesthetic and sedation if appropriate.
If you are having mons pubis liposuction, the surgeon will insert a small cannula into the treatment area, loosen the fat cells and suck them out using a special vacuum device.
For skin excision, the surgeon will make an incision in your lower abdomen, in the part which is usually covered by bikini bottoms, and cut away any excess skin with a scalpel. The cuts are closed with dissolvable stitches. This means you will not have to return to the clinic to have the stitches removed.
Following your mons lift surgery, you will recover in our ambulatory recovery rooms. You may feel a little drowsy but this often wears off within a few hours. Once you are able to eat and drink, and our specialist nursing team are happy that your initial recovery is complete, you will be allowed to leave the clinic with a friend or family member.
04.Treatment Recovery
After surgery, once the anaesthetic has worn off, it is normal to experience some discomfort, bruising or swelling in the mons pubis area. This may last for two to three weeks. Any pain can be easily controlled using over-the-counter painkillers. You should notice the results of your mons pubis surgery immediately.
One or two days after your mons surgery, you will be mobile and should be taking slow-paced walks. If you have a desk job, or a job which is not very physical and doesn’t involve heavy lifting, then you can usually return to work after a week. You can resume taking baths three weeks after your mons pubis reduction and you may start exercising again between four and six weeks after surgery. You should expect a full recovery by six weeks.

The Patient JourneyWhat is the mons pubis?
The mons pubis is a mounded area of fatty tissue that covers the pubic bone. Although both sexes have mons pubis, it is far more prominent in women.
The mons pubis is shaped like an inverted triangle and stretches from the top of the natural pubic hair line down to just above the clitoris. The mons pubis is naturally covered in pubic hair. Inside the mons pubis there are glands which secrete pheromones, which are responsible for sexual attraction. The mons pubis also acts as a protective ‘cushion’ during sexual intercourse.
Why do I have an oversized mons pubis?
There are a number of reasons why someone may have an oversized mons pubis.
If you gain weight, excess fat tends to accumulate in the mons pubis, causing it to bulge and enlarge and stretch the covering skin. When this weight is lost, without enough elasticity, the excess skin doesn’t return to its normal size and sags. Sometimes this tendency to accumulate fat in the area is inherited from your parents.
Women who have previously undergone a C-section may find themselves left with a bulging mons pubis. This is because after a C-section the skin around the scar may stay right against the tissues, but this is not the same for the skin below it.
Oestrogen can cause increased fat deposition in your mons pubis, hence the mons pubis is naturally more prominent in women.
What other treatments can I combine with mons lift?
Mons pubis reduction can be carried out as a stand alone procedure or in conjunction with other cosmetic surgery procedures. It is common to have mons reduction alongside a tummy tuck (abdominoplasty) or it may be a component of mummy makeover surgery. A mummy makeover is tailored to meet the needs of the individual patient and typically involves procedures that target the breasts, abdomen, waist, hips, thighs and vagina.
You could also combine mons surgery with gynaecological procedures, such as vaginoplasty, labiaplasty or vaginal rejuvenation.
Should I have mons surgery during a tummy tuck?
Mons lift surgery is commonly performed in conjunction with a tummy tuck (abdominoplasty).
When a person has lost large amounts of weight, a lot of loose, sagging skin is quite often left behind. A tummy tuck removes this excess skin and the muscles and fascia of the abdominal wall are tightened to achieve a tighter, flatter-looking stomach. Mons reduction is often carried out at the same time as an enlarged mons pubis is more noticeable once the abdomen has been tightened and sculpted. Our expert surgeon will be able to discuss this combination of treatments with you at your consultation.
How long does it take to recover from a mons lift?
It takes six weeks to make a full recovery from mons pubis reduction surgery.
In the immediate aftermath of mons surgery, you may experience some discomfort and bruising or swelling which may last for up to three weeks. You should be reasonably mobile within a day or two of surgery. You can return to work after one week, although you may need an extended period off work if your job is physically demanding or involves heavy lifting. Exercise, including going to the gym, can be resumed between four to six weeks.
It may take longer to recover if you are combining your mons reduction combined with other surgeries or treatments.
What are the side effects and risks of a mons lift?
Although mons reduction is a safe and low-risk surgery, there are risks associated with liposuction carried out on any area of the body. Common side effects include bruising, swelling, temporary numbness, inflammation and fluid coming from incisions. Very rarely, liposuction can cause; lumpy or uneven results, bleeding under the skin, persistent numbness that can last for months, changes in skin colour, build-up of fluid in the lungs, a blood clot in the lungs or damage to the internal organs.
Any type of operation also carries a risk of excessive bleeding, developing a blood clot in a vein,
infection or an allergic reaction to the anaesthetic. However the risk of any of these happening during your treatment is small.
Is there any scarring following mons reduction?
If you have mons pubis liposuction, then this leaves minimal scarring behind. For skin excision, a small cut will be made across the bikini line, which will result in a small scar. However this scar will be hidden by clothing. If there is a large amount of excess skin to remove, then a vertical cut may be necessary.

How soon after mons surgery can I resume sexual activity?
You should refrain from pentrative sexual activity for one month after your mons reduction surgery. This is because the mons pubis acts as a ‘cushion’ during sexual intercourse and any friction can exaccerbate discomfort, bruising and swelling in the area. You should only return to sexual activity when you feel ready to do so. If you find that sexual intercourse causes pain or discomfort of the mons pubis, stop and try again after more time has passed.
