What is a Brazilian Butt Lift?

A Brazilian butt lift, often known just by its initials BBL, is a highly popular cosmetic procedure which involves a fat transfer to the buttocks to create a fuller bottom.

The Brazilian Butt Lift, or BBL, sees the patient’s own fat cells harvested from another part of the body and treated before being reintroduced to the buttocks. The result is a perkier, more rounded, behind. By using the patient’s own fat, this means our Brazilian butt lift procedure produces a natural-looking, and natural-feeling, result.

Brazilian butt lift surgery has enjoyed a boost in popularity in recent years, driven by celebrities such as Kim Kardashian. But the procedure has often made the headlines for the wrong reasons, as a BBL can be dangerous when carried out by underqualified and inexperienced surgeons. However, if the surgery is carried out by a qualified and experienced surgeon at a reputable clinic, BBLs which utilise fat transfer to the buttocks are as safe as any other cosmetic surgery.

Some clinics use silicone enhancements or insert foreign material into the buttocks. This can be dangerous and often produces substandard results, as this material is at risk of slipping or moving out of place. Here at the Cadogan Clinic, we do not use any foreign bodies in our Brazilian butt lifts.

Other benefits to using the patient's own naturally-occurring fat resource include a quicker and less painful recovery time, less scarring and a lower risk of potential complications. Utilising fat transfer to the buttocks also allows the surgeon to create bigger, more shapely, buttocks if so desired by the patient. Silicone inserts do not allow for this flexibility.

Women opt for Brazilian butt lift surgery if they are unhappy with the shape and size of their buttocks. What is involved in a Brazilian Butt Lift?

Brazilian butt lift surgery can be broken down into three different stages:

Before your treatment, you will decide with your surgeon where the fat cells to be used for your BBL are to come from. Donor sites may include your abdomen, thighs or hips. In liposuction, the fat cells are broken up by way of vibrations or a high-pressure water jet and a small incision is made for a cannula to be inserted into the donor site. Sometimes more incisions may be required. A vacuum machine is used to loosen and suck out the fat. This part of the process will result in a reduction of fatty deposits in the donor area, leaving the area more sculpted.

In the second stage of the Brazilian butt lift procedure, special equipment is used to eliminate any impurities, preparing the fat to be reintroduced to the body. The fat is placed in a centrifuge which spins to separate the fat from any oil, blood and excess fluid. The amount processed depends on how much volume you have decided with your surgeon that you want to be introduced into the buttocks. Once this fat processing phase has been completed, the process moves on to the third and final stage.

Once the fat has been treated, it can be put back into the body via a cannula which is specially designed for buttock augmentation. The fat will be introduced into the buttocks above the muscle. The surgeon will then proceed to sculpt the area, to create the larger, more rounded, buttocks required.

What is involved & Patient Safety

In order to uphold the highest safety standards, here at the Cadogan Clinic we use ultrasound-guided fat injection during Brazilian butt lift procedures.

The surgeon will use an ultrasound probe to identify the different anatomical layers of the buttocks. Transferred fat is introduced into the subcutaneous layer of the skin. This is the layer of skin directly below the dermis and epidermis. As the treated fat is introduced, the process is displayed on a screen. This allows for improved accuracy and avoids the possibility of any fat going into the muscle. When inexperienced surgeons place fat straight into the muscle, this can cause serious health problems and sometimes even death. The Cadogan Clinic surgeons are experts in their field and we use the latest technology and surgical techniques to prevent injury or any complications from occurring.

The procedure takes about 3 to 4 hours to complete on average depending on the amount of liposuction performed. This varies depending on the number of donor areas being used and whether any other surgical procedures are being carried out alongside the Brazilian buttock lift.

When it comes to extracting the fat, a good rule of thumb is to extract twice the amount needed for the transfer, to allow for the small amount of fat which will not survive the transfer process.

We do not use implants or any other non-organic material in any of our Brazilian butt lift procedures here at the Cadogan Clinic as we believe they produce sub-standard results and put the patient at a greater risk of complications.

By using the patient’s own natural fat (autologous fat), this means the body will not reject the fat and won’t cause any immune reaction.

Recovery from fat transfer to the buttocks is swift and yields results quickly. You should expect some swelling and bruising in a couple of weeks following your Brazilian butt lift, and you may experience some pain and discomfort. However, you should expect to have recovered from surgery within four to six weeks.

Brazilian Butt Lift and safety

When introducing the harvested fat to the buttocks, the fat should be placed under the skin through a cannula and never straight into the muscle. However under-qualified surgeons and physicians have simply injected fat straight into the buttocks without understanding the anatomy. Injecting fat into the muscle causes a fat embolism to enter the blood through large veins in the muscle of the buttocks. This fat then travels to the heart or lungs, causing respiratory distress and death.

There have been incidents where people have been left seriously injured after travelling abroad for Brazilian buttock lift surgery, tempted by cheap prices. However, undergoing a cheap cosmetic procedure at the hands of an under-qualified surgeon or in a substandard facility can result in huge physical and financial complications once a patient has arrived back in the UK. In 2018 a British woman sadly died during Brazilian butt lift surgery in Turkey when a fat clot blocked one of her arteries.

What are the benefits of BBL?

There are a number of benefits to having Brazilian butt lift surgery.

By using the body’s own natural fat cells and not introducing any foreign objects into the body, a BBL procedure produces a natural-looking result while adding a more rounded shape. If you do go on to lose weight after you have recovered from surgery, through visits to the gym, for example, your body shape will change accordingly, rather than remain static. The likelihood of complications is also much reduced by using the body’s own fat, rather than a silicone insert.

As we age, sagging and a loss of shape in the buttock area is a common problem. A buttock augmentation will restore and enhance the curvaceous shape of the buttocks.

A Brazilian butt lift can help to improve a person’s bodily proportions. If you have a ‘top heavy’ body, a Brazilian buttock lift can help to give you an hourglass shape.  A BBL is also a great way of improving the appearance of the buttocks if a person is unable to achieve this by exercise alone.

With age, weight gain and after having children, women’s bodies can lose the definition and natural curves they enjoyed previously. This in turn can have an impact on the fit and appearance of clothing. A Brazilian butt lift, coupled with the contouring of the BBL donor sites, can enhance body shape, meaning clothing such as jeans and dresses appear much more flattering. Brazilian buttock lift surgery leaves minimal scarring, so you can also wear a bikini confident in the knowledge that there will be no signs of surgery visible.

Buttock augmentation will leave the bottom and thighs looking more firm and toned. Lifting the buttocks can have an impact on the appearance of cellulite, making it much less visible.

As well as improving the size, shape and volume of the buttocks, Brazilian butt lift surgery can also improve other areas of the body too. In order for fat to be transferred to the buttocks, excess or unwanted fat cells must be harvested from other areas of the body, such as the hips, love handles, thighs or abdomen. This means that as well as enhancing the appearance of the buttocks, a BBL procedure can also help to sculpt other areas of the body, transforming the figure.

Brazilian butt lift surgery, like other forms of cosmetic surgery, can have an overwhelmingly positive impact on a person’s mental health and outlook. What to expect

Below you will find some key information to help you prepare for surgery and make necessary arrangements to ensure you have the support you need and that everything runs smoothly on the day.

Your journey with the Cadogan Clinic will start with up to two consultations with one of our leading specialist plastic surgeons at our award-winning premises in Chelsea. These consultations will take between 30 and 60 minutes.

During your consultation, your surgeon will ask about your expectations for your BBL surgery and talk about what will happen during the procedure. They will also explain how the procedure will affect you.

It is important that you are open and honest about what you hope to achieve through surgery to help ensure the best possible result. Your surgeon will be happy to answer any questions you may have at this point. It may be a good idea to write your questions down before attending the consultation. Having established what you want from the surgery, and what is possible given your anatomy, several key measurements will be made and standard clinical photography will be taken. Your medical history will be discussed and a physical examination will be carried out.

If your surgeon feels you are a good candidate for surgery, you are then invited to consider whether you would like to proceed with your Brazilian butt lift or not following a two week 'cooling off' period.

If you decide to go ahead with surgery following the cooling-off period, the next time you will return to the clinic is on the day of the procedure. It is important that you prepare for your buttock augmentation by taking the following measures ahead of surgery.

Stop smoking: Smoking decreases blood flow in the skin, meaning that less of the transferred fat to the buttocks is likely to ‘take’ and establish a new blood supply. This will have an impact on the overall result of your BBL. If you smoke, your doctor will recommend that you stop smoking six weeks before surgery and during the recovery period.

Cut down on alcohol: Excessive drinking can hinder the healing process.

Avoid taking certain medications: You should avoid taking aspirin, anti-inflammatory drugs and herbal supplements in the week ahead of your Brazilian butt lift. Taking these can increase bleeding.

Arrange for help during recovery: Make plans for a friend or family member to drive you home after you leave the hospital and stay with you for at least the first night of your recovery at home.

In the six hours prior to surgery, you must not consume food or any drink, other than small sips of clear fluid which will be allowed up to two hours before you are admitted to the clinic. Clear fluids include still water, black coffee and black tea.

On the day of your procedure, we ask that you arrive for your admission an hour before the agreed start time of surgery. At this point a nurse will come and record blood pressure and other relevant tests, you will meet with your anaesthetist and your surgeon who will make the final mark-ups. A general anaesthetic is administered. Here at the Cadogan Clinic, we use Total Intravenous Anaesthesia (TIVA) to ensure minimal sickness after the BBL procedure and a quicker recovery time. 

The Brazilian butt lift surgery takes in the region of three to four hours to complete.

Your surgeon will make small incisions on the liposuction areas and the fat tissue is removed using specialised cannulas. This harvested fat is then treated before being reintroduced to the buttocks, adding volume to the posterior. The volume of fat transferred depends on whether the patient wants to achieve increased projection of the buttocks or to fill in ‘hip dips’ - depressions that occur just below the hip bone. The incisions are closed with sutures.

Following your Brazilian butt lift, you will recover in our ambulatory recovery rooms. You may feel a little drowsy but this often wears off within a few hours.  Once you are able to eat and drink, and our specialist nursing team are happy that your initial recovery is complete, you will be allowed to leave the clinic with a friend or family member.

You may feel some pain or discomfort following your Brazilian butt lift surgery, but this can be controlled using over-the-counter painkillers.

You will be up and mobile within a day of your buttock augmentation and will be able to wash in three days.

You should avoid sitting on your buttocks for two weeks and avoid sleeping on your back for two weeks following surgery to allow for the buttocks to settle into their new shape and avoid flattening. A post-operative garment must be worn after BBL surgery.

You will meet in person with our nursing team one week after surgery to check the incision sites. We also recommend meeting one of our aestheticians to discuss ongoing treatment to support the healing process if required.

A final check-up will take place six weeks after your Brazilian butt lift.

There are a number of factors that will determine the overall cost of your BBL. This includes the number of areas which are having liposuction and whether any other surgical procedures will take place alongside your BBL.

It takes six weeks to fully recover from Brazilian buttock lift surgery. 

We recommended taking a week off work following your buttock augmentation. You should be able to return to work and your day-to-day activities within five to seven days after surgery.

It will take four weeks until you can drive again. We recommend avoiding sexual activity until four weeks post-surgery.

Exercise, including going to the gym, should be avoided until the six-week mark.

Newly transferred fat is delicate and requires time for a new blood supply to be established to the fat cells. Some of this fat, between 20 and 40 per cent, will be what is called ‘non-viable’ and safely absorbed into the body. This figure can be as high as 50 to 70 per cent in smokers, so we strongly urge you to quit ahead of your BBL surgery. 

The process to establish a new blood supply can take between three and six months, when the final results of your buttock augmentation can be seen.

It is vital that you follow all the aftercare advice issued by your surgeon in order to ensure a quick recovery while maintaining the result of your surgery. 

You will be left with some scars following your Brazilian butt lift procedure. However these are minimal and are easily hidden, so you will be able to wear swimwear and similar items of clothing without the worry of scars being easily seen by others.

The incisions required for a fat transfer to the buttocks are only small as only fine, specialised cannulas are used in this procedure. This means that the patient is left with minimal scarring. Our expert surgeons will be able to hide any scarring in the natural folds and contours of the body.

We would recommend quitting smoking and cutting down excess alcohol consumption ahead of surgery. Smoking and alcohol can interfere with wound healing. This in turn can leave the wound open to infection and increases the likelihood of scarring.

Wearing a compression garment will hold the wound together, allowing it to heal. This helps to further reduce the risk of scarring following the BBL procedure.

Your incision sites will be checked one week and six weeks after your Brazilian butt lift procedure. If you have any concerns about healing, you can call our 24/7 on-call service for advice. This aftercare is included in the cost of your treatment here at the Cadogan Clinic.

You will need to wear a postoperative surgical garment for six weeks following your Brazilian butt lift. The reason for this is to help minimise swelling and keep the buttocks in their new shape, preventing flattening. By holding the wound together, the surgical compression garment also helps to promote healing and reduce the likelihood of infection.

A compression garment made for use following Brazilian butt lift surgery uses light lycra material to protect the operation area, providing vital compression, supporting the back and thighs and encouraging lymphatic drainage. Despite sounding quite severe, many patients find these compression garments comfortable and they can be discreetly worn underneath clothing. Garments are available in various shapes and sizes to best suit your body and its needs. Your surgeon will be able to advise you in more detail about compression garments.

Failure to wear a surgical garment may cause the buttocks to lose their new shape and lead to a substandard result, which may require a revision procedure.

Brazilian butt lift surgery produces permanent results and the volume enhancement will not reduce over time. By taking care of your new body, you can enjoy the results of your BBL for many years to come.

However, your body will still be at the mercy of the ageing process. As we age, the skin loses collagen and elastin, which gives the skin its youthful volume and elasticity. As these levels deplete, the skin starts to wrinkle and sag. This includes the skin on the buttocks. At this stage, you may wish to consider further surgical treatment.

A BBL will not stop the ageing process, but it can turn the clock back, meaning the process will resume at a more advantageous starting point. However, any changes will be minor.

However dramatic weight gain and weight fluctuations can have a negative impact on the results of your Brazilian buttock lift, so it is important that you eat healthily and exercise in order to maintain the result for as long as possible.

It is possible to see results immediately from Brazilian buttock lift surgery.

However, a Brazilian butt lift is an invasive cosmetic surgery, meaning it is normal to experience bruising and swelling following a BBL. This will take a few weeks to subside. You should expect to see a final result once all the post-operative swelling has disappeared, at around the three-month mark. It may take up to six months to see the final result of your Brazilian butt lift procedure. Once again, we would like to stress that you must follow all the post-operative results issued by your surgeon. Failure to do so may slow down the healing process.

Sometimes not all of the fat transferred to the buttocks will establish a blood supply and may be absorbed into the body. This means there may not be as much volume present in the buttocks as hoped. If this is the case, you may wish to discuss the possibility of further BBL procedures with your surgeon.

What are the risks?

The Brazilian butt lift procedure is not without its risks, and you should always bear the potential risks in mind before opting to undergo any aesthetic surgery. Possible risks and complications of the Brazilian buttock lift include:

  • Haematoma (blood collecting under the skin)
  • Death of fat tissue (fat necrosis)
  • Poor wound healing
  • Asymmetry (one buttock looking larger or different to the other)
  • Seroma (a lump caused by a build-up of fluid) 
  • Poor graft take

A poor result may require a revision procedure. The surgeons here at the Cadogan Clinic are experts in their field and have carried out Brazilian butt lift procedures many times. However, a revision procedure may sometimes be required for reasons beyond the surgeon’s control.

In addition to this, any type of surgery carries a small risk of:

  • Excessive bleeding (haemorrhage)
  • An allergic reaction to the anaesthetic
  • Infection at the surgical site
  • Deep Vein Thrombosis (blood clot in the vein)
